- Changes in appetite
- Changes in water intake
- Changes in weight
- Missed doses, with dates
- Doses that may have been doubled, with dates
- New symptoms and when they started
- Anything that changed after a medication started or stopped
- Who gives the medications at home
- Any trouble giving a medication
Questions to ask
- Are all of these medications still needed?
- Is there anything I should not give along with them?
- What should I do if I miss a dose?
- What should I watch for at home, and when should I call?
- When is the next check-up or test?
- Who do I call after hours?
