A one-page pet vaccination record: one place to write down each vaccine, the date it was given, when the next one is due, and which clinic gave it. It is handy when you board your pet, visit a groomer, sign up for daycare or a class, move, or change vets. Your vet decides which vaccines your pet needs and when. This sheet only records what your vet's paperwork says.

How to fill it in
Ask your clinic for a copy of your pet's records, or use the certificates and invoices you already have. Write each vaccine name exactly as it is printed, then the date given, then the next due date as your vet's record states it. Add the clinic name in the fourth column if your pet has seen more than one. If you have a rabies tag number or a certificate number, copy it into the box at the top so you can find it later.
When people ask for proof
Boarding kennels, groomers, daycares and training classes often ask for proof of vaccination, and the rules differ from place to place. Ask each one what they accept. A tidy summary helps you answer quickly, but many places will still want the official record from your vet, so keep those originals and ask your clinic whether they can email a copy.

Your vet's records are the official ones. This sheet is a working copy for your own use: a quick way to see what your pet has had and what is coming up. If your copy and the clinic's records ever disagree, the clinic's records win, so check with them.
Bring it to the vet
Bring the sheet to each visit, especially the first one at a new clinic, and ask them to check it against their records. It sits well next to a medical history form and a vet visit checklist. For medications you give at home, a medication log records each dose.

Is this an official vaccination certificate?
No. It is a personal record. Ask your clinic for an official certificate when a kennel, groomer, airline or landlord asks for one.
Which vaccines should I list?
List whatever your vet has given. This page does not say which ones a pet needs. That is your vet's decision, and it can depend on your pet, your area and your lifestyle.
Can I use it for cats and other pets?
Yes. Write the species at the top and list whatever your vet has recorded.
